CenterPoint's Medicaid Application

On April 1, 2011, the NC Department of Health and Human Services issued a request for applications (RFA) to solicit applications from Local Management Entities to operate a 1915 (b)/(c) Medicaid waiver. CenterPoint applied to become a Medicaid waiver site and was granted approval in October of 2011. CenterPoint will be relying heavily on community input and collaboration to implement waiver on January 1, 2013. CenterPoint's originally submitted Medicaid Application Documents can be found below.
