Public Records Request

All public records requests are to be submitted to the Marketing and Public Relations Officer at 4045 University Parkway Winston-Salem, NC 27106. Requests can also be made by emailing cphs@cphs.org. Public records requests submitted over the phone will not be accepted as it is CenterPoint's policy to retain a written request.
